Here at Consortium Chartered Physiotherapists we are pleased to be able to offer home visit rehabilitation sessions across Hull, East Yorkshire and North Lincolnshire.
These offer patients with mobility difficulties or chronic conditions convenient and personalised care with treatment plans tailored to the home environment.
At home patients can benefit from being in a more comfortable and less stressful setting. This can support recovery and encourage adherence to the exercise plans and rehabthat our team put in place.
Treatment plans can be tailored to the specific home setting and where required can focus on practicing real life tasks. We aim to support patients in achieving realistic, functional goals that have the potential to enhance their quality of life. This could be getting out of bed, into the car, walking to the shop or reaching items in the kitchen.
We also encourage family involvement in the treatment plan if desired and are very happy to communicate regularly and flexibility with caregivers.
Our home visit process consists of an initial assessment from one of our consultant physiotherapists. At this session an agreed plan is discussed and provisionally put in place. Following a decision by everyone to go ahead, regular treatment sessions will then be carried out with one of our senior rehabilitation therapists. The frequency and length of these session is flexible and can be decided according to the patient’s needs.
The price of a home visit session from our team varies depending on the patient’s location and proximity to our clinic. The base price for each session is listed below and additional costs will be added to the total to account for travel.
Base price:
Initial Assessment with senior physiotherapist £70.00 (45 minutes)
Rehabilitation session with senior rehab therapist £55.00 (45 minutes)
(Additional travel and time costs from the clinic are calculated on providing the home address postcode)
